Immunization Information For All RHS & BMS Students to Meet New York State Department of Health Requirements
All children in the following grades need to have physicals for this school year:
All students new to the district
All students participating in sports
THESE CHILDREN ARE REQUIRED BY NEW YORK STATE LAW TO HAVE A PHYSICAL.
If you would prefer to have your child’s exam done by his/her own doctor, it must:
* Have been done no more than 12 months prior to the start of the school year.
* Be completed and signed by the examining physician.
* Returned to the school nurse as soon as possible.

- However it is not necessary to use only this form, providing the physician includes what is required by this form, including the BMI and Weight Status Category.
If the completed form is not on file, the exam will be done at school when the rest of your son/daughter’s class is scheduled. (Sports Physicals are offered periodically and may require the student to sign up in the health office-unless they will have a physical done by their own physician.)
Please note that physicals are good for one calendar year. (However, following serious illnesses or injuries, reassessment of health and written documentation by a physician may be required.)
Please note the following required immunizations for school:
3 doses of DPT,
Tdap for students entering 6th grade- discuss the specifics with your physician.
3-4 doses of OPV, IPV or any combination of OPV and IPV,
3 Hepatitis B, or 2- if adult dosage is specified by the physician,
2 doses of Varicella (chicken pox) for students born on or after 1/1/94 entering or transferring into) the 6th grade, and students in gradeless classes born on or after 1/1/94 and age equivalent of the 6th grade. (Diagnosis by a physician of a student having had chicken pox is acceptable proof of immunity).
1 dose of Meningococcal vaccine-for students entering grade7, (they will be required to get a booster at age 16). 2 doses for Students entering grade 12-unless the first dose was given on or after age 16
Documentation of the above immunizations must be signed or stamped by a physician.
